Abstract

In order to assess the need for the home health care, the survey was performed for the hospitalized patients with chronic diseases in the tertiary institutions from September 25th to October 15th.
The total sample consisted of 210 patients with various diagnosis such as cancer, renal disease, diabetes mellitus, cardiovascular & pulmonary diseases, musculoskeletal diseases, gastrointestinal diseases and others.
The data analysis was divided into four sections. In the first section, socio-demographic features of the sample were described. In the second section, the need Intensity and contents for the home health care were identified. In the third section, the relationship between characteristics of the patients and need Intensity for the home health care was analyzed with the ANOVA test. In the last section, the determining factors on the need intensity for the home health are was examined through the stepwise regression.
As a result, the finding of the analysis were as follows :
(1) The sample mean age was 62 years. More than 60 % of the patients were married. The majority of patients were living in Seoul with their families. A spouse or child was most often named as the primary caregiver. The living standard of the sample was in the middle class or over. More than 70C) of the sample had a admission experiene. The functional status of patients was almost independent.
(2) The order of the contents the patients in the category of the nursing assessment was as follows : medical assessment(57.6 %), measurement of vital sign and blood pressure( 57.1 %), skilled physial assessment (44.8 %), specimen collection (40.0 %), and so on : in the category of the nursing intervention ; health counseling, guidance, and health teaching(61.4 %), adminstration of medication(54.8 %), administration of IM or subcutaneous or IV (48.6 %), demonstration of proper caring (41.9 %), coordination of medical services( 36.7 %) , etc.
(3) Regarding to the need intensity for the home health care, more than 70 % of the patients showed comparatively strong need.
(4) The hypotheses based upon the third research goal were verified with the ANOVA test as follows :
1) There was a relationship between age and need for the home health care(p<0.05).
2) There was no relationship between admission experience and need for the home health care (p> 0.05).
3) There was a relationship betwee I level of education and need for the home health care(p< 0.05).
4) There was a relationship marital status and need for the home health care(p<0.05).
5) There was a relationship between living standard and need for the home health care(p< 0.05).
6) There was no relationship between accessibility to the medical services and need for the home health care (p > 0.05) .
7) There was a relationship between functional status and need for the home health care(p< O. 05).
(5) The stepwise regression analysis indicated that residence, marital status, and primary caregiver(in this order) influenced on the ,need intensity for the nursing assessment(26 %) residence, occupation, and age influenced on the need intensity for the nursing intervention(50 %) : occupation, primary caregiver, age, residence, and admission experience influenced on the need intensity for the overall nursing activities(49 %).
